Transitions is an adult day service program that is activities based. It offers an individualized schedule of groups and activities designed to help the person served develop or maintain skills for future vocational placement.

The Transitions Program operates between Monday-Friday from 8:00A.M.-3:00P.M. at Blick Clinic 682 West Market Street. Daily schedules are determined individually within that time frame, based on need. Although program enrollment is expected to be short-term or temporary, there is not a specified time limit and length of stay is individually determined.

Transitions is for individuals who may be between jobs or work placement. Individuals may be stepping down from Partial Hospitalization treatment, lost their job due to reduction in force or program closing, recently entered adult services or have a part time work schedule and need additional day programming.
