COMMUNITY SUPPORT PROGRAM

The Community Support Program is designed to assist individuals in addressing a variety of challenges associated with mental illness.  The program provides services in the most appropriate and supportive environments, with an emphasis on optimizing the individual's adaptation to his or her environment as a whole.  A contract with the Summit County Alcohol, Drug Addiction, and Mental Health Services Board (ADM Board) provides these supports to over 70 adult individuals.  Those desiring more details regarding referral to the Community Support Program should contact the ADM Board.

SPECIFIC SERVICES

The Community Support Program provides services in the following areas:

  • On-going assessment of individual strengths and needs.
  • Assistance in achieving/maintaining personal independence as desired by the individual or guardian.
  • Development of current and desired daily living skills.
  • Coordination of the Individual Service Plan.
  • Monitoring of mental health symptoms.
  • Crisis management/coordination.
  • Education as appropriate for the individual and/or family regarding assessed needs and strengths.
  • Interventions addressing symptions, behaviors, etc. that eliminate barriers to optimizing individual life potential.
  • Facilitation of activities that increase the individual's ability to positively impact his or her own environment.
